Travel Vaccination

Due to an increased workload and demand for our Practice Nurse time we are not able to offer advice regarding travel vaccinations.  You can request a printout of your vaccination history from reception (please allow 48 hours before collecting).

You will be informed of any vaccinations you require including those available on the NHS. 

Cholera (in special circumstances), Tetanus, Diphtheria, Polio, Hep A and Typhoid are available to travellers as part of the NHS regime, therefore if you need a booster of any of these, we can provide this.

Please remember to leave enough time to have your immunisations prior to travelling - at least three months is advisable.

Offical Forms

There are a variety of forms which require a doctor's signature e.g. Passport application, private medical forms etc. The doctor will complete these for you but please allow at least three working days for completion and there may be a fee. A list of fees is available in the waiting room, from reception or on our website in the following links. Please note the Practice is registered for VAT and therefore some forms may be subject to VAT. The Practice accepts payment by Visa/Mastercard/Maestro debit and credit cards. There is a 50p charge for credit card payments.
